I have someone who I have preapproved for a weekend yet another group is requesting the same weekend but for an extra day, which would be beneficial. However, I don’t think I can offer anything after preapproval the first group? Don’t I risk getting a negative rating? And once I preapproved someone, don’t they have first right of refusal?

You will not be penalized for withdrawing a pre-approval. It is not the same as declining a Request to Book. Airbnb does track how many times you decline a Request to Book. Even then, you have to decline "alot" according to Airbnb for it to hurt you in search rank.

I usually don't suggest my Host clients use pre-approvals. The booker many times doesn't have the guest count correct, or dates as they are just inquiring about your property and may have multiple requests in the system for multiple properties. You can pre-approve several guests at the same time, but you see that having multiple pre-approvals out at the same time can get confusing and you might have a better booking opportunity from another guest as you found out.

Pre-approval is not a reservation.
When you pre-approve someone, it just means they can instantly book within the time window Airbnb gives them (usually 24 hours). If they don’t confirm and pay, the dates remain open to others.
If another group sends a booking request for the same dates and they actually confirm first, Airbnb will block the calendar for them. It’s “first to book, first to get it.”
