This is my first time hosting and I have a nice young couple from France. Nice when I see them, that is. Twice now they have holed up in the room for 18 straight hours, and now 20 straight hours. The first time I heard them leave to go to the bathroom a few times. Right now I haven't heard them do that, but it is a bit louder out here.
Still, at a certain point should I text or knock on the door to see if they're alive? I have a mini fridge in there, but can't imagine what they're eating really.
Is this normal? Why would you come visit NYC and spend the whole time in the room?

Maybe they have jet-lag or one (or both!) of them is sick? I'd send them a message via AirBnB asking if everything is ok.
Well, they spent 18 hours straight over New Years Eve into New Years Day. I just messaged them and it turns out they snuck out in the wee hours and are currently in Washington, DC. Yay! So much better than just about anything else they could have gotten into in their room all day. Worries averted!
I once had a booking from a young man from Notre Dame. He was traveling with his girlfriend, an international student from Korea. This was when we started, and we were hosting in our home, in the spare room. They arrived, made small talk and then I went to run errands. They were in their room the ENTIRE time, leaving only to use the restroom and have a shower. The one time they left their room, they must have thought nobody was home and they were making out on the couch in the common living area. When we came upstairs and suprised them, they almost had a heart attack. They stayed with us for 6 days, and went into the city once, for a total of four hours. When they checked out, the bed was broken (young acrobatic love). I don't get why they would come all the way to New York to hole up in a room, they could have gotten themselves a motel 5 miles off campus and saved the money!
I also wondered at some points in they were alive. I guess they just managed their time efficiently so that they quickly did everything they needed to while we were out shopping or going for a walk.

Since we also share a space with our guests we've deviced a system to let us know if guests are home or not. We ask them to leave the keys in the hanger when they are here. That way we know if they are home so we can "behave". Since we have two cats we ask guests to keep their doors shut at all times so the key is really what tells us if they are here or not. Also, at another Airbnb, I've seen a little sign with "guests are" and another with "in/out". Also "hosts are" and another with "in/out". That helps THEM know if YOU are home 🙂

We have 2 guests ona five day booking. They have only come out once at night for 2 hours. The weather is beautiful. What could they be doing? Should I be concerned?
Personally no - as it sounds like you have been in the house for 5 days too! Joking in a way, but perhaps they are having a family meeting, praying, doing something unremarkable like making a gigantic jigsaw puzzle.
