Rating Calculation

Can anyone provide answers to any of the following 3 questions regarding ratings for reviews?

 

Q1 - My five star rating is listed as 69%.  However I counted 140 fives out of a total of 172 stars, which is 81.4%.  How was the 69% calculated?

 

There were 29 reviews, but the stats at the top indicate there were 31 reviews.  What happened to the other two?

 

All 29 reviews showed 6 categories, except for one that showed only 4 categories.  Why did it not show the other two categories?
