Automated Guest Arrival Form made on Guesty showing up blank to guest on Airbnb?

Hi all, 

I use Guesty for Hosts to manage my listing. I have my first guest arriving in 7 days and I scheduled a Guest Arrival Form to be sent out automatically a week before guest check in. This morning the guest messaged me saying the form was coming up blank when she clicked on it. I went back and when I am on Guesty I can click the link and pull up the form with no issue. 

I went on the Airbnb website and clicked on the form and it is coming up blank. What am I doing wrong?
