Cannot load payout information

Hello,

I've been trying to add a payout method for the past couple days.  I followed the steps as the process is direct and user friendly.  However, the site simply "grays out" all the fields on the last page.  I've entered my bank info several times, backed out, waited for hours, e.t.c.  

Is it that I'm adding a savings account?

I've looked in the help section, I've scanned the site and cannot find anywhere some sort of tech support link, chat, or email.

Any ideas would be helpful.
