The Guest that booked the reservation is ill, but would like for her family to use the reservation?
I have a guest that has reached out stating that she is ill and will not be able to check in this weekend. She is requesting that I still allow her family to use the home. This has never happen and although I believe her story, I'm a little leary. How do I handle this?

I would Phone air bnb asap, explain the situation and see if you can cancel the reservation without financial or other penalties to your listing and let this guest make a NEW booking under the correct guest ID profiles of who is staying on this new visit.
